Accredited Board Governed Licensed and Certified Providers may apply for in-network status with Upper. Approval is contingent on the list below.

Natural-First Service/Product Alignment
Upper will verify your businesses values and natural-first treatment and therapies alignment through research, past customer/patient reviews, website and social media marketing and messaging, secret shopping, and any other reasonable means necessary to identify proper cultural fit.
Board Governed Licensing or Certification
Accredited and Board Governed Licensing or Certification is required. College and University Degrees without official licensure or certification do NOT qualify. For example, a Nurse Practitioner is “Licensed,” while a Personal Trainer is “Certified”. Both types of legitimate accreditation qualify for partnership under their own scope. Upper will use reasonable means to verify the legitimacy, status, activity, and historical records of the businesses’ employee’s license(s) and/or certification(s), including but not limited to State Government license lookup and search tools, State requests for copies of licenses/certifications, Board or Organization license/certification lookup and search tools, email requests to such organizations for direct verification, etc.
Licenses and Certifications will be compared to application services and products being offered. Scope of license/certification must legally match services/products being offered.
Malpractice Insurance
Proof of Malpractice or Professional Certification of Insurance (“COI”) is required, which must provide minimum coverage of $1M. Property insurance is not required and will not be considered.
Photo Identification
Photo identification, such as a Driver’s License, or other means of ID, is required. Information on the uploaded ID will be compared and verified against the Insured Party’s information, and Licensures and Certifications. Applicant information must match within reasonable means (i.e. a Business Owner who employs Providers may apply, where the licenses, certifications, services must be uploaded. The Insured business or party must insure the named and licensed employees.
